American Health Holding (AHH)—the 4th District Health Fund’s case management provider—helps 4th District participants and their covered family members with complicated health issues.

What is Case Management?

Case management is a confidential service that is available at no cost to you if you have an illness or injury that is difficult, long-term or costly. AHH case managers are specially trained registered nurses and licensed social workers. They will work closely with you, your family and your providers to help ensure that you get high quality, cost-effective care. Your case manager can answer your questions and will help you understand your treatment and options for your care.

How Do I Enroll in Case Management?

There are multiple ways for you to become involved in case management. AHH may reach out to you based on precertification requirements. In addition, you, a family member or a provider can call AHH at 1-800-641-3224 to determine if you would benefit from case management.
